Ascendance Truck Centers Photography and Media Consent Form

Ascendance Truck Centers ("the Company") occasionally use employee photographs and videos for marketing, promotional, and informational purposes. These materials may be used on the Company’s website, social media channels, print materials, internal communications, and other marketing platforms. By signing this form, you provide your consent for the Company to capture, use, and distribute your image as described below.

1. Consent to Use I, the undersigned, hereby grant Ascendance Truck Centers permission to use my likeness in photographs, video recordings, and other media. I understand that these materials may be edited, copied, exhibited, published, or distributed for the following purposes:
   • Marketing and advertising materials
   • Social media posts and updates
   • Website and blog content
   • Internal and external communications
   • Trade shows or events
2. Rights Granted I understand and agree that:
   • The Company owns all rights to the photographs and media content created.
   • My likeness may be used in conjunction with other visuals, text, or designs.
   • The media may be stored indefinitely unless I revoke this consent in writing.
3. Revocation of Consent I understand that I may revoke this consent at any time by submitting a written request to Ascendance Truck Centers’ Human Resources Department. Revocation will not apply to materials already published or in use prior to the receipt of the request.
4. Release of Liability I release Ascendance Truck Centers, its employees, agents, and representatives from any claims, demands, and liabilities related to the use of my image or likeness.
5. Voluntary Agreement I understand that my participation is voluntary and that I am not required to provide consent as a condition of my employment.
6. Waive Rights I waive any right to royalties or other compensation arising from the use of these materials.
